Immunization Requirements for
Attending Schools in the State of Alabama

Alabama schools must ensure that each pupil presents a valid Certificate of Immunization as required by the Code of Alabama, 16-30-4.  A child’s entry and attendance in school is conditioned upon the presentation of a current and valid Alabama Certificate of Immunization unless a recognized exemption applies.  Exemptions include Medical and Religious Exemptions, and for those deemed Homeless under Federal Law. 

  • Medical Exemption – Issued by the child’s Primary Care Physician or Specialty Physician
  • Religious Exemption – Issued by the Alabama Department of Public Health. Contact them for further information
  • Homeless under Federal Law – St. Clair County Schools will work with the parents/guardians to assist in acquiring immunization records

Students may register/enroll but must not attend school until the requirement is met.

Students currently enrolled and their immunization record has expired will have 30 days to present the school with a current Certificate of Immunization or the student will be withdrawn from school.

 

How to obtain an Alabama Certificate of Immunization

IF YOUR IMMUNIZATION STATUS IS UP TO DATE

According to the Alabama State requirements, meaning no immunization is required at present, the St. Clair County Health Department will transfer your immunization dates for ALL students onto a State of Alabama Immunization form.  You will need to contact them to set up an appointment.  You must provide proof of identity (Driver’s License. Birth Certificate, Medicaid or Medicare Card, Employee/School Identification Badge, Social Security Card) and your immunization Record/Information.

IF YOUR IMMUNIZATION STATUS IS NOT UP TO DATE

If your child has insurance, please see your child’s health provider, a local Walk-In Clinic for an appointment.  Fees are determined by the provider.   Immunization availability may vary by location.  Contact them for more information.

If your child has no health insurance or has Medicaid, please contact the St. Clair County Health Department for more information and to set up an appointment.  The hours of operation are Monday – Friday from 7:30 am – 5:00 pm.
